Signs that a knee replacement may help you
A knee replacement is usually considered when arthritis has badly damaged the joint and it is limiting your life, despite proper non-surgical treatment. You may benefit if several of these apply to you:
- Knee pain that limits walking to a few hundred metres, or makes stairs difficult
- Pain at rest or at night that disturbs your sleep
- Difficulty getting up from a chair, using an Indian toilet or sitting on the floor
- A knee that is bending out of shape: bow legs (varus), knock knees (valgus) or a knee that will not straighten
- Stiffness, swelling or the knee giving way
- Little or short-lived relief from medicines, physiotherapy, weight loss and injections
- X-rays showing advanced (grade 4) arthritis, with bone rubbing on bone
Not sure? Take the 60-second Knee Test. If several answers are “No”, get your knee evaluated.
Who is a good candidate?
Osteoarthritis, rheumatoid arthritis or arthritis after an old injury that has worn out the joint.
Bow legs, knock knees or a fixed bend, where precise correction matters most.
Sugar, blood pressure, heart and lungs controlled; checked before surgery.
Willing to follow the physiotherapy plan, with family support at home.
When surgery may NOT be necessary
Dr. R. K. Singh recommends surgery only when it is the right choice. Surgery is usually not the first step if:
- Your arthritis is early or moderate on X-ray and pain comes and goes
- You have not yet tried a proper course of physiotherapy, weight management and medicines
- The pain comes from somewhere else: the back, hip, nerves or a soft-tissue problem around the knee
- A recent injury (ligament or meniscus) can be treated on its own, sometimes by arthroscopy
- Pain is mild and you can still do what matters to you
- An uncontrolled medical condition or an active infection needs treatment first
In these cases the plan may be stage-wise arthritis treatment: strengthening physiotherapy, weight control, medicines, a knee support, and PRP or gel injections where suitable, with a review over time.
Stage of arthritis and the usual approach
| Stage | What it usually means | Usual approach |
|---|---|---|
| Early (1–2) | Mild wear, pain after activity | Exercise, weight control, activity changes |
| Moderate (3) | Frequent pain, swelling, stairs difficult | Physiotherapy, medicines, injections where suitable |
| Advanced (4) | Bone on bone, deformity, pain at rest | Knee replacement, often Zero Error and robotic |
The decision depends on examination, X-rays and how much the knee limits you, not on the stage alone.
Common questions
Am I too old for knee replacement?
Age alone is rarely the reason to say no. Many patients in their 70s and 80s do well after knee replacement when their heart, lungs, sugar and blood pressure are controlled. What matters is your overall fitness, which is checked before surgery.
Am I too young for knee replacement?
Younger patients are usually treated without surgery for as long as it works. If the joint is badly damaged and pain is limiting work and life, a knee replacement can still be the right choice; modern implants are designed to last many years.
My X-ray shows arthritis. Does that mean I need surgery?
No. X-rays show the damage, but the decision also depends on your pain, function and how you respond to other treatment. Many people with arthritis on X-ray manage well without surgery.
Can injections or physiotherapy avoid surgery?
In early and moderate arthritis they often reduce pain and improve movement for months or longer. In advanced, bone-on-bone arthritis with deformity they usually give only short relief.
Should both knees be operated together?
If both knees are badly affected and you are fit, both can often be replaced in one sitting, with one hospital stay and one recovery. Otherwise they are done a few weeks apart. Dr. Singh advises after checking your health.
